We had over 30,000 failed registrations this year.
For various reasons.
The most interesting part: 90% of them tried to register with Hotmail accounts. :|
Obviously, Hotmail (Microsoft) has problems with their servers.
Also, many accounts have wrong (too high) Anti-Spam settings.
I cannot help these guys.
For them, the best option is to use another email provider (Yahoo, Gmail, etc).
If they don't receive the activation email, they can send an email to
necromanthus at yahoo dot com
They must include the desired username and password.
